Related to (r/t) (etiology): lack of social support
As evidenced by (aeb): verbalization of loneliness, void of spouse, no family in town, desire for more contact with others, depressive features
Outcome: Resident will attend one or more social activities during my shift.

Assess Clients perception of loneliness. Among persons with severe mental illness, more than half identify problems with loneliness. (A &L 534) Findings have related loneliness in the elderly to depression (THA pg 631)
Assess The client’s ability/inability to meet psychosocial, physical, spiritual, and financial needs and how unmet needs further challenge the ability to be socially integrated. The client’s perception of general health and social support influences health status outcome. (A&L 534) Increased aging may lead to loss of independence. (THA 633)
Do Find ways to increase client’s support system and participation in groups and organizations. Support systems promote self-esteem and emotional health (A &L 535) Active participation in groups is essential for behavioral changes. (Psychiatric Mental Health 279)
Do Encourage client to engage in meaningful relationships and support personal attributes. Personal attributes and social support mediated the effects of emotional distress by decreasing its impacts on ADLs. (A&L 535) Primary relationships are intimate associations that provide a strong sense of sharing and belonging. (THA 491)
Teach positive use of solitude to prevent loneliness. Reading, music and art are activities that enhance well being and decreased feelings of loneliness. (A & L 536) Music plays a role in preventing the feeling of loneliness (Psych. Mental 494)
Teach Family members to express caring by including the client’s in their lives by letting them know where they’ll be and how to reach them. Loneliness can only be alleviated and made less painful. T his can only be achieved by increasing the awareness of this distressing condition. (A&L 535) Develop an caring and sharing network within the group(family) (THA 501)
